Health Extension for Diabetes




Developed in 2017, Health Extension for Diabetes (HED) is a diabetes self-management support program created by Clemson University and recognized by the American Diabetes Association as a practice-tested diabetes support program.

HED Structure

Participant Registration

Find eligible clients through referrals and participant signups to create HED cohorts.

Educational Sessions

Facilitators lead 8 education sessions on topics ranging from "Eating Healthy with Diabetes" to "Reducing Risks with Diabetes".

Support Sessions

Facilitators lead intermittent support sessions designed to individualize support for participants and provide them with the necessary skills to successfully self-manage their diabetes.

4

Graduation

Upon successful completion of the program, participants have an opportunity to celebrate finishing the program at Graduation.
